Please print, complete, initial and sign these two forms prior to your first visit.  Bring them with you to your appointment.

If you have a pain condition, please print and complete this Pain Questionnaire, and bring it with you to your appointment.

If you do not have access to a printer, please notify me before your appointment.  I will ask you to arrive early, so that I may bring the necessary forms to your parked vehicle.  You will need complete them before entering my office.

Wear clothing that is loose and comfortable. During the warm months, shorts and a tank top (or no top for men) is appropriate. During the winter, soft and loose clothing that can be rolled up above your knees and elbows works well. Stiff or tight jeans, belts and bulky clothes interfere with the treatment.  You may bring a change of clothing if you are coming from work.

If you have recently exercised, please shower before your appointment. 

Please do not wear perfume out of consideration for those with respiratory allergies.

Please bring a kitchen-size plastic garbage bag, similar size gym bag or tote bag to your appointment.  All your personal belongings must go in this bag.

Bring a fresh mask, hand sanitizer, your own pen and water bottle to your appointment. 

When you arrive, text or call me, and I will come out and meet you in the driveway.  Please allow me three minutes to come out before calling again.

Before entering please remove all jewelry and wrist watches, and empty your pockets of cell phone, wallet, etc.  You may place these personal items in your bag which you can bring into my office.  After you’ve locked your car door, please apply hand sanitizer to your hands.  You will be given more hand sanitizer once inside the treatment room.

Please notify me before your appointment if you have shortness of breath, fever, cough, runny nose, sore throat, loss of taste or smell, or you feel like you might be getting sick.  Please notify me if you have travelled in the past 2 weeks, attended any mass events, or come into contact with a Covid-19 patient or with anyone exhibiting signs of acute respiratory infection.

It is best to arrive for your appointment not too hungry and not too full.  Eating a light meal or snack before your appointment is fine.

I have a 24-hour cancellation policy.  If you need to cancel or change your appointment time, notify me at 617-964-9519 at least 24 hours beforehand.  There is no cancellation charge if you feel like you may be developing a cold or flu, or if you’ve been exposed to a sick or coughing person since you scheduled your appointment.
